Choose the right care

A known recurrence or a new problem?

For a Virginia adult without an easy appointment or insurance coverage, the first decision is whether a known cold-sore recurrence can be reviewed online. The second is which care setting fits the budget and the problem. A $45 online assessment may be useful for an established recurrence; a first or uncertain lesion needs an examination rather than a low-price shortcut to antivirals.

Online review may fit

  • You are 18 or older and physically in Virginia.
  • A healthcare provider previously diagnosed oral herpes or cold sores.
  • This looks like your usual recurrence and you meet the safety limits.
  • You can answer follow-up questions through the portal.

An exam or another care pathway

A first or uncertain mouth sore needs an examination. Pregnancy or breastfeeding, significant immune suppression, kidney disease needing adjusted dosing, or an antiviral allergy require another care pathway.

A genital-herpes diagnosis alone does not establish an oral-herpes diagnosis. This visit does not provide new herpes diagnosis or STI testing.

Clear costs

What does cold sore treatment cost in Virginia?

$45Clinician assessment and visit follow-up messaging.
Medication is paid separately to the pharmacy.
Bidwell does not bill insurance for the visit.

A transparent fee still needs a useful comparison. Bidwell is $45 for review, with medication paid separately. A Virginia clinic may have assistance or insurance arrangements, but eligibility and availability differ. Ask about examination and testing charges before choosing; no competitor fee or total medication price is assumed on this page.

Ask the pharmacy for the price of your exact medicine, strength and prescribed quantity. Cash prices, insurance coverage, discounts, brand availability and delivery fees can differ. The herpes treatment cost guide explains the separate visit and pharmacy charges; its examples are not a quote for your pharmacy.

Useful local resources

Virginia resources when cost is a barrier to an examination

Free, charitable and community clinics

The Virginia Health Care Foundation's medical-care resource points to free and charitable clinics and community health centers. The foundation explains that it supports these providers rather than delivering care itself. Contact the clinic you select to ask about income or insurance eligibility, the services offered and appointment availability.

The Enroll Virginia clinic directory provides a map and listings of free clinics and community health centers. Its guidance distinguishes the eligibility rules of a free clinic from possible lower-cost care at a community health center. Use the listing as a starting point, not a promise of a free appointment or free medication.

Ask for an exam that fits the lesion

Explain whether the problem is on the lip, inside the mouth, at a mouth corner or close to the eye. Tell the office if it is the first episode or has not healed as expected. A medical or dental examination may be more appropriate than requesting a repeat cold-sore prescription.

If the clinic cannot arrange the assessment you need promptly, ask about an appropriate alternative. Severe symptoms follow the urgent and emergency guidance below. Do not delay necessary care while waiting to qualify for a particular payment program.

From Northern Virginia to a temporary stay elsewhere in the state

You can request a Bidwell assessment while physically in Virginia if you meet the prior-diagnosis and safety requirements. Being a Virginia resident is not enough while you are elsewhere. Washington, DC, North Carolina and West Virginia are not in Bidwell's current service area; Maryland is supported, but the visit must record that actual location.

If you are staying away from home in the Shenandoah Valley, Hampton Roads or another part of Virginia, list a pharmacy you can reach during your stay. Ask the branch about stock, dispensing hours and the full prescription charge. A clinician's decision and a pharmacy's readiness are separate steps.

For a repeat outbreak, share when the symptoms began and what you previously took. If you already have an active prescription with authorized refills, the pharmacy can tell you whether a refill is available. Do not take another person's medicine or use leftover tablets to cover a different-looking lesion.

Frequent episodes deserve a plan that accounts for their effect on your life. Ask about episodic treatment and whether a discussion of off-label suppression makes sense. A new visit does not guarantee a daily prescription or a particular duration of treatment.

Treatment decisions

Can I get valacyclovir or Valtrex online?

Eligible patients can request assessment for oral valacyclovir, the generic medication in Valtrex, or an acyclovir alternative. A clinician chooses treatment after review. Oral acyclovir for cold sores is off-label; famciclovir and topical acyclovir are not offered through this visit.

Early symptoms matter, but the timing needs context

Tell the clinician when tingling started and whether blisters have appeared. The US Valtrex label says effectiveness when started after visible cold-sore signs develop has not been established. That is not a promise of benefit at any stage or proof that every later presentation is untreatable. Read the treatment-timing guide.

Antivirals do not cure herpes. Daily cold-sore suppression is off-label and a separate clinical decision. Follow your own prescription; do not borrow tablets, extend a course or use a genital-herpes regimen for a cold sore on your own.

Could it be something other than a cold sore?

Cold sores, also called herpes labialis, are caused by herpes simplex virus. A recurrence may start with familiar tingling or burning before blisters appear on or around the lips. Appearance alone cannot confirm an unfamiliar diagnosis.

Canker sore
An ulcer inside the mouth rather than a typical lip-border recurrence. A large, persistent or recurring ulcer needs medical or dental assessment, particularly after two weeks.
Impetigo
Leaking sores with honey-colored crust can reflect a bacterial infection. An examination is needed; a herpes antiviral does not treat impetigo.
Angular cheilitis
Cracks at the mouth corners may have causes other than herpes. Persistent or recurring cracks need assessment.
Irritation or allergy
Lip products or toothpaste can irritate the skin. Get assessment if the problem is uncertain, persistent or worsening. Swelling with breathing difficulty is an emergency.

How your online visit works

  1. Share the history. Give your prior diagnosis, symptom timing, medicines, allergies, physical location and chosen pharmacy.
  2. Respond to the clinician. Check portal messages. Review may lead to more questions, an examination recommendation or a prescription when appropriate.
  3. Confirm the pharmacy fill. If prescribed, check availability, total cost and directions with the pharmacy. Bidwell does not sell or ship medication.

When to seek urgent or emergency care

Get urgent same-day care for eye symptoms, sores near the eye, severe swelling, trouble swallowing or drinking, or fever with severe illness. Severe swelling with breathing difficulty is an emergency: call 911. Confusion, new weakness, or a severe headache with a stiff neck, especially with fever, also require emergency care. Call 911 if you cannot get there safely; do not wait for an online message response.

Widespread or rapidly spreading blisters, especially with eczema or a weakened immune system, also need urgent in-person assessment. Do not wait for an online review.

Cold sore care in Virginia: common questions

Can I use Bidwell without insurance in Virginia?

Yes. The visit is cash-pay at $45 and Bidwell does not bill insurance. Medication is paid separately. If you need an exam and cost is a barrier, the Virginia clinic resources above may help you find another option.

Are the listed Virginia clinics always free?

No. Eligibility, services and payment arrangements vary. Contact the selected clinic. A directory listing does not guarantee a free visit, medication assistance or an immediate appointment.

Can I request care from Washington, DC with a Virginia address?

No. Washington, DC is outside Bidwell's current service area. Physical location during care determines eligibility, not a home address or pharmacy selection.

Does a first cold sore qualify for this visit?

No. Bidwell requires a prior oral-herpes or cold-sore diagnosis from a healthcare provider. A first or uncertain sore needs an in-person examination and any testing the evaluating clinician recommends.

Can I get cold-sore tablets over the counter?

Oral valacyclovir and acyclovir require a prescription. Some nonprescription cold-sore products are available, but they do not establish a diagnosis or replace an exam for an unfamiliar sore. Ask a pharmacist which product is appropriate for your situation.

Is it too late if a blister has already appeared?

Early treatment is preferred. The US Valtrex label says effectiveness when started after visible cold-sore signs develop has not been established. Tell the clinician the timing and current appearance rather than assuming treatment must work or cannot help.

How quickly will my visit be reviewed?

Reviewed 7 days a week, including weekends; timing varies. Additional questions and pharmacy processing can affect the time to treatment. A submitted visit does not guarantee an immediate prescription or same-day pickup.

What if the clinician says I need in-person care?

You receive a full refund when a clinician determines that your case cannot be safely or appropriately treated through Bidwell Health, including when we refer you to in person care. Other refund terms and exceptions apply; read the full terms before starting.

Reduce the chance of passing it on

Avoid kissing and oral sex from the first tingling until the sore has healed. Do not share lip products or kiss a baby while you have a cold sore. Wash your hands after touching it. HSV can spread without visible sores, and medication does not make contact risk-free.
