Choose the right care
A known recurrence or a new problem?
If reaching your usual clinic in Montana takes planning, first decide what the visit needs to do. Reviewing a familiar outbreak after an established oral-herpes diagnosis may be possible online. Examining a first sore, a new rash or a lesion that has changed cannot be replaced by convenience. The goal is to avoid an unnecessary trip without delaying a necessary one.
Online review may fit
- You are 18 or older and physically in Montana.
- A healthcare provider previously diagnosed oral herpes or cold sores.
- This looks like your usual recurrence and you meet the safety limits.
- You can answer follow-up questions through the portal.
An exam or another care pathway
A first or uncertain mouth sore needs an examination. Pregnancy or breastfeeding, significant immune suppression, kidney disease needing adjusted dosing, or an antiviral allergy require another care pathway.
A genital-herpes diagnosis alone does not establish an oral-herpes diagnosis. This visit does not provide new herpes diagnosis or STI testing.
Clear costs
What does cold sore treatment cost in Montana?
Medication is paid separately to the pharmacy.
Bidwell does not bill insurance for the visit.
A $45 assessment and a pharmacy fill are separate costs. For a long trip, also consider whether a community clinic could provide the examination you actually need. Ask that clinic about its payment arrangements; neither the directory nor this page promises a free visit, a particular medicine price or a ready-to-collect prescription.
Ask the pharmacy for the price of your exact medicine, strength and prescribed quantity. Cash prices, insurance coverage, discounts, brand availability and delivery fees can differ. The herpes treatment cost guide explains the separate visit and pharmacy charges; its examples are not a quote for your pharmacy.
How does this compare with other care?
| Option | Useful when | What to check |
|---|---|---|
| Bidwell online visit | A familiar recurrence after a prior diagnosis, within the service limits. | $45 review; separate pharmacy charge; no exam or immediate-fill promise. |
| Primary care or community clinic | You need an exam, continuity of care or broader medical assessment. | Appointment availability, insurance or self-pay cost, and any testing fees. |
| Urgent care | You need a prompt in-person assessment and your usual office is unavailable. | Services offered, availability and complete visit charges. |
| Another online service | You are comparing a different consultation format or treatment scope. | Who reviews the visit, diagnosis limits, subscription terms and total charges. |
For emergencies, seek emergency care. Do not choose a routine service based on price.
Useful local resources
Montana: plan the right trip, not just the shortest one
Find a center that offers the service you need
The Montana Primary Care Association health-center directory helps you identify local care. Look up a center near your location, then check whether the relevant site offers medical care, dental care or both. A clinic network's full range of services may not be available at every branch.
When you call, describe the problem clearly: a familiar lip recurrence, an unfamiliar mouth ulcer, a spreading rash or pain with drinking. Ask whether an examination can be arranged, what the appointment will cost and what information to bring. This is especially useful before setting aside time for a longer drive.
Keep the clinic trip and the medicine trip separate
An online assessment may avoid travel to the clinician, but you still need a workable way to obtain any prescribed medicine. Check the pharmacy's actual dispensing hours, stock and fill estimate before you leave. If the first branch cannot fill it, contact the care team about the appropriate next step.
For severe symptoms, do not wait to combine a clinic appointment with other errands. Eye symptoms, trouble drinking, severe illness or spreading symptoms need the urgent assessment described below. If safe transport is unavailable in an emergency, call 911.
What to arrange before submitting from Montana
Have a way to read portal messages after you request care. Message-based review does not require a scheduled video appointment, but the clinician may need an answer before deciding. If you are about to travel somewhere with limited connectivity, consider how you will receive and respond to those questions.
Write down when the tingling or burning started and when a visible sore appeared. Mention whether the previous diagnosis came from a healthcare provider and whether this episode matches that pattern. If you are unsure of the diagnosis, the local examination pathway above is the more appropriate starting point.
For future familiar recurrences, ask your clinician whether an advance treatment plan is appropriate. That is an individual prescribing decision, not a standing promise of medication. If you already have a plan and labeled medication, follow its instructions instead of improvising an extra course or borrowing someone else's tablets.
Your Montana location still matters during review. Crossing into Idaho, Wyoming, the Dakotas or Canada does not preserve eligibility because your home address is in Montana; those are outside Bidwell's current service area. Keep the care team informed if travel changes where you are.
Treatment decisions
Can I get valacyclovir or Valtrex online?
Eligible patients can request assessment for oral valacyclovir, the generic medication in Valtrex, or an acyclovir alternative. A clinician chooses treatment after review. Oral acyclovir for cold sores is off-label; famciclovir and topical acyclovir are not offered through this visit.
Early symptoms matter, but the timing needs context
Tell the clinician when tingling started and whether blisters have appeared. The US Valtrex label says effectiveness when started after visible cold-sore signs develop has not been established. That is not a promise of benefit at any stage or proof that every later presentation is untreatable. Read the treatment-timing guide.
Antivirals do not cure herpes. Daily cold-sore suppression is off-label and a separate clinical decision. Follow your own prescription; do not borrow tablets, extend a course or use a genital-herpes regimen for a cold sore on your own.
Could it be something other than a cold sore?
Cold sores, also called herpes labialis, are caused by herpes simplex virus. A recurrence may start with familiar tingling or burning before blisters appear on or around the lips. Appearance alone cannot confirm an unfamiliar diagnosis.
- Canker sore
- An ulcer inside the mouth rather than a typical lip-border recurrence. A large, persistent or recurring ulcer needs medical or dental assessment, particularly after two weeks.
- Impetigo
- Leaking sores with honey-colored crust can reflect a bacterial infection. An examination is needed; a herpes antiviral does not treat impetigo.
- Angular cheilitis
- Cracks at the mouth corners may have causes other than herpes. Persistent or recurring cracks need assessment.
- Irritation or allergy
- Lip products or toothpaste can irritate the skin. Get assessment if the problem is uncertain, persistent or worsening. Swelling with breathing difficulty is an emergency.
How your online visit works
- Share the history. Give your prior diagnosis, symptom timing, medicines, allergies, physical location and chosen pharmacy.
- Respond to the clinician. Check portal messages. Review may lead to more questions, an examination recommendation or a prescription when appropriate.
- Confirm the pharmacy fill. If prescribed, check availability, total cost and directions with the pharmacy. Bidwell does not sell or ship medication.
When to seek urgent or emergency care
Get urgent same-day care for eye symptoms, sores near the eye, severe swelling, trouble swallowing or drinking, or fever with severe illness. Severe swelling with breathing difficulty is an emergency: call 911. Confusion, new weakness, or a severe headache with a stiff neck, especially with fever, also require emergency care. Call 911 if you cannot get there safely; do not wait for an online message response.
Widespread or rapidly spreading blisters, especially with eczema or a weakened immune system, also need urgent in-person assessment. Do not wait for an online review.
Cold sore care in Montana: common questions
Can an online visit save me a drive to a Montana clinic?
It may avoid travel for assessment when this is a familiar, previously diagnosed cold sore and you meet the safety limits. It cannot replace an exam for a new or uncertain lesion, and any prescribed medicine still needs to be filled.
What if I cannot check portal messages until tomorrow?
A clinician may need more information before deciding. Review and pharmacy timing vary, so plan to check messages and respond. If you need urgent assessment, use in-person care instead of waiting for an online response.
Can you guarantee the pharmacy has valacyclovir?
No. Stock, dispensing hours and the fill estimate come from the pharmacy. Ask the specific branch and contact the care team if a change is needed after a prescription is sent.
Can I request a plan before my next outbreak?
You can discuss future recurrent-outbreak treatment with the clinician. The decision depends on your diagnosis and health history. Advance prescribing is not guaranteed and does not replace an exam for unfamiliar symptoms.
Can I get cold-sore tablets over the counter?
Oral valacyclovir and acyclovir require a prescription. Some nonprescription cold-sore products are available, but they do not establish a diagnosis or replace an exam for an unfamiliar sore. Ask a pharmacist which product is appropriate for your situation.
Is it too late if a blister has already appeared?
Early treatment is preferred. The US Valtrex label says effectiveness when started after visible cold-sore signs develop has not been established. Tell the clinician the timing and current appearance rather than assuming treatment must work or cannot help.
How quickly will my visit be reviewed?
Reviewed 7 days a week, including weekends; timing varies. Additional questions and pharmacy processing can affect the time to treatment. A submitted visit does not guarantee an immediate prescription or same-day pickup.
What if the clinician says I need in-person care?
You receive a full refund when a clinician determines that your case cannot be safely or appropriately treated through Bidwell Health, including when we refer you to in person care. Other refund terms and exceptions apply; read the full terms before starting.

Reduce the chance of passing it on
Avoid kissing and oral sex from the first tingling until the sore has healed. Do not share lip products or kiss a baby while you have a cold sore. Wash your hands after touching it. HSV can spread without visible sores, and medication does not make contact risk-free.
